Since skin disease not a leading cause of mortality it is not usually thought of as a major health issue in developing countries.  However, it is extremely common up to 21-87% of the population may be affected by some type of skin problem, depending on the locale.  It also contributes to significant physical and psychological disabilities. These disabilities have been measured by a number of life quality measures but the importance of skin health is often underestimated because of the chronic, non-life threatening nature of most skin diseases. 1.Skin and Immunology

The skin is an immunological organ, consisting of a variety of specialised cellular and soluble mediators, including skin cells (keratinocytes), Langerhans cells, dermal dendritic cells, resident T cells, monocytes,

complement, antibodies, and antimicrobial peptides. The skin holds an estimated 20 billion T cells! The diverse and functionally specialized components of the immune system sense and respond to infection or various barrier breaches to activate an immune response and eventually return to homeostasis (steady state). However, poorly regulated immune responses can also cause inflammatory skin disorders.  Skin infections result from inadequate immune control of colonising microbes. The skin is lined by many organisms (fungi, bacteria, viruses), and disturbances in the flora or the immune system’s response to these bugs can cause disease.

2.Skin Disorders

Skin disorders vary greatly in symptoms and severity. They can be temporary or permament, and may be painless or painful. Some have situational causes, while others may be genetic. Some skin conditions are minor, and others can be life-threatening. Many temporary skin conditions exist, including Contact Dermatitis and Keratosis Pilaris. Some chronic skin conditions are present from birth, while others appear suddenly later in life. The cause of these disorders isn’t always known. Many permanent skin disorders have effective treatments that enable extended periods of remission. However, they’re incurable and symptoms can reappear at any time. Examples of chronic skin conditions include: Rosacea, Psoriasis and Vitiligo.

3.Eczema

Eczema/dermatitis has the symptoms of itching, reddening, scaling, and edematous papules, and the condition progresses in a specific inflammatory reaction pattern. Eczema/dermatitis is histopathologically characterized by intercellular edema called spongiosis, which can be caused by extrinsic factors, such as irritants or allergens, or by intrinsic factors, such as atopic diathesis. These factors interact in complex ways, and extrinsic and intrinsic factors are seen together in many cases.  If the cause is not identified, eczema may be called acute, subacute or chronic, depending on the clinical and pathological features. Eczema usually affects the face, elbows and knees.

4.Skin Allergy

Allergy   is an exaggerated inflammatory response is mounted against a harmless substance. The most common skin allergies include eczema, hives/angioedema and contact dermatitis. Allergic skin disease occurs when the body’s immune system over-reacts to some offending substance. Substances to which animals are allergic ("allergens") produce additive effects, so the severity of the itchy skin lesions is a result of the summation of each allergen’s effect. The most common causes of allergic skin disease are inhalants (such as pollens, molds and house dust), foods and fleas. Inhalant allergies (called "atopy") and food allergies most commonly affect the face, ears, front of the forelegs, webbing between the toes of the front paws, belly, groin and armpits. Other factors which can exacerbate allergic skin disease include overly dry skin (for example, secondary to hypothyroidism), sub-optimal diet, flea infestation and  skin infections (for example, infected skin folds in heavily wrinkled dogs). Other causes of skin diseases, including parasites (e.g., mites), fungi (e.g., ringworm), autoimmune reactions and endocrine disorders, should be treated.

5. Skin Cancer

Skin cancer must starts in the cells of the skin. Some other types of cancer start in other parts of the body and can spread to the skin, but these are not called skin cancers. Skin cancers are named for the type of cells that become cancer.

Cancers that develop from melanocytes, the pigment-making cells of the skin, are called melanomas. Skin cancers that are not melanoma are sometimes called non-melanoma skin cancersbecause they tend to act very differently from melanomas. The 2 most common kinds are:

  • Basal cell carcinoma
  • Squamous cell carcinoma

Melanoma: Melanoma is a cancer that starts in skin cells called melanocytes. Melanomas can grow anywhere on the skin, but are more likely to start in certain locations like Trunk (men), Neck, Legs (women) and Face.

Basal cell skin cancers: They usually develop on sun-exposed areas, especially the head and neck.

Squamous cell skin cancers: Tend to be more aggressive than basal cell cancers. They commonly appear on sun-exposed areas of the body such as the face, ears, neck, lip, and back of the hands. They can also develop in scars, chronic skin sores, or in a pre-cancerous lesion called an actinic keratosis. Less often, they form in the skin of the genital area.

6.Bacterial Skin Infections

Bacteria are microscopic, single-cell organisms that live almost everywhere. A bacterial skin infection is a proliferation of a harmful strain of bacteria on or inside the skin. Bacterial skin infections are usually caused by gram-positive strains of Staphylococcus and Streptococcus or other organisms.

Bacterial skin infections often begin as small, red bumps that slowly increase in size. Some bacterial infections are mild and easily treated with topical antibiotics, but other infections require an oral antibiotic. 7.Viral Skin Diseases

Viral skin infections can produce localized or disseminated lesions. Viruses can cause skin rashes and painful inflammation. They range from the common to the rare, from the mild to the severe and from those causing just skin infection to those with associated systemic disease. Most viral skin diseases present with an exanthem (skin rash) and, oftentimes, an accompanying enanthem (lesions involving the mucosal membrane)Different types of viral infections include: Herpes simplex, Herpes zoster, Molluscum contagiosum, Warts, Measles, German measles (rubella), Chickenpox (varicella virus) and Primary HIV infection (often associated with a rash).

8.Fungal/Yeast Skin Infections

These types of skin infections are caused by a fungus and are most likely to develop in damp areas of the body, such as the feet or armpit. Some fungal infections aren’t contagious, and these infections are typically non-life-threatening.

These types of skin infections are caused by a parasite. These infections can spread beyond the skin to the bloodstream and organs. A parasitic infection isn’t life-threatening but can be uncomfortable.

10.Noncancerous, Precancerous and Cancerous Tumors

Tumours are groups of abnormal cells that form lumps or growths. They can start in any one of the trillions of cells in our bodies. Tumours grow and behave differently, depending on whether they are cancerous (malignant), non-cancerous (benign) or precancerous.

Cancerous tumours: Cancer can start in any part of the body. When cancer cells form a lump or growth, it is called a cancerous tumour. A tumour is cancerous when it:

  • grows into nearby tissues
  • has cells that can break away and travel through the blood or lymphatic system and spread to lymph nodes and distant parts of the body

Non-cancerous tumours: Tumours that aren’t cancerous are called non-cancerous tumours. Non-cancerous tumours:

  • stay in one place and don’t spread to other parts of the body
  • don’t usually come back after they are removed
  • tend to have a regular and smooth shape and have a covering called a capsule

Precancerous conditions: Precancerous cells are abnormal cells that may develop into cancer if they aren’t treated. Some of these cells have mild changes that may disappear without any treatment. But some precancerous cells pass on genetic changes and gradually become more and more abnormal as they divide until they turn into cancer. It can take a long time for a precancerous condition to develop into cancer.

Precancerous changes can be mild to severe.

11.Diseases of Pigment

Disorders of pigmentation present as skin that is discolored, blotchy, or darker or lighter than normal. They occur when the body produces too little (hypopigmentation) or too much (hyperpigmentation) melanin. Melanin is a pigment that creates hair, skin, and eye color and protects the skin by absorbing ultraviolet light. These disorders can be localized or can diffusely spread about the body. Some pigmentation disorders, such as liver spots, are common, whereas others, such as albinism, are rare, affecting approximately 1 out of every 17,000 people. With some disorders, the cause of dyspigmentation may be readily identified as sun exposure, drug reactions, or inflammation; in other cases, the etiology is not as clear. 12.Sexually Transmitted Diseases (STDs) and Skin Condition

Sexually transmitted diseases (STDs) are infections that are passed from person to person by sexual activity. Although STDs are most often spread through vaginal, anal, or oral intercourse, skin-to-skin contact is enough to transmit many STDs.

Microorganisms that exist on the skin or mucus membranes of the male or female genital area can be transmitted, as can organisms in semen, vaginal secretions, or blood during sexual intercourse. There are many types of STDs, and all have slightly different symptoms. STDs that cause prominent skin changes are discussed here. For example:

Genital warts, caused by the human papillomavirus (HPV), and molluscum contagiosum appear as small, painless bumps or growths.

Genital herpes (herpes simplex virus), primary syphilis, chancroid, and lymphogranuloma venereum (LGV) cause open sores, blisters, or ulcerations in the genital region.

Pubic lice and scabies mites can cause itchy, red bumps in the genital region.

Gonorrhea can cause a discharge from the vagina, penis, or rectum.

13.Bites, Infestations, Papules, Scales, Plaques and Eruptions

Bug bites, stings, puncture wounds, and insect infestations can cause serious health complications.

A papule is a solid elevation of skin with no visible fluid. It can be brown, purple, pink or red in color, and can cluster into a papular rash. Papules may open when scratched and become infected and crusty.There are many skin diseases which develop papules, such as Lichen planus, a skin disease which classically forms polygonal, purple papules.

Several skin disorders and physical conditions can lead to scaling skin. Scaling skin is usually a symptom of an underlying issue. Related conditions and diagnoses may include:

Actinic keratosis, allergic eczema, skin ulcers, athlete’s foot, dermatitis, contact dermatitis, hypoparathyroidism, ichthyosis vulgaris , ringworm, psoriasis etc.

A plaque is a flat, primary lesion. Plaques can appear anywhere on the body, but they appear most often on the elbows, knees, scalp and lower back.

15.Autoimmune Diseases of Skin

Autoimmune diseases are a type of condition in which the body’s immune system begins to attack and terminate healthy cells and tissues within the body. The underlying cause of autoimmune diseases is not fully known. Many autoimmune diseases affect the skin: noticeably changing coloring, moisture and photosensitivity.

20.Medical Treatments- Emerging & Future Therapies

Alopecia areata, atopic dermatitis and vitiligo are highly visible dermatologic conditions that can have a negative effect on patients’ quality of life and overall health. An emerging treatment option, however, could provide effective therapy for patients with these conditions. It is an exciting time in dermatology, as there continues to be a shift in the understanding of the pathophysiology of many disease states, bringing new therapies for difficult to treat conditions. Ameluz (aminolevulinic acid hydrochloride)(Biofrontera AG) is a new drug for treatment of mild to moderate actinic keratosis on the face and scalp; Taltz (ixekizumab)(Eli Lilly and Company) for the treatment of moderate to severe plaque psoriasis; Cosentyx (secukinumab)(Novartis Corporation); Cotellic (cobimetinib)(Genentech USA, Inc); Imlygic (talimogene laherparepvec)(Amgen, Inc) is a modified oncolytic viral therapy; Kybella (deoxycholic acid)(Allergan) is a nonhuman, nonanimal, synthetically created compound; Odomzo (sonidegib)(Novartis Corporation) for locally advanced basal cell carcinoma were approved by FDA in last three years.

There are many new effective medications that have only recently hit the market that require our attention, as either FDA approved for the intended dermatologic use or could be effective in treating conditions that previously have been poorly managed.

21.Emergency Dermatology

Outside dermatology, the term “dermatologic emergency” is often regarded as an oxymoron. Unless a patient’s skin is sloughing off — as in Stevens-Johnson syndrome/toxic epidermal necrolysis. People who don’t typically see a dermatologist may not think to call one if a strange rash develops. About 50% of patients with true dermatologic emergencies go straight to an ER. But since the majority of cutaneous outbreaks are not life-threatening. Most emergent rashes are caused by medication. Even common exanthematous drug rashes can progress to life-threatening SJS or TEN if the offending medication is not identified and halted.
